ADVERSE LICENSING ACTIONS - 1999CACIOPPO, Dino T., M.D.: License # 009651; 03/23/99. In lieu of a hearing, the licensee voluntarily surrendered his license to practice medicine in the State of Maine. Unprofessional conduct, guilty plea to misdemeanor sexual assault on a female patient, based on California Board action. HEILNER, David P. Jr., M.D.: License # 012267; 04/13/99. In lieu of a hearing, the licensee voluntarily surrendered his license to practice medicine in the State of Maine. Licensee may not reapply for licensure until one (1) year after the date of Consent Agreement. Unprofessional conduct, incompetence and violation of the Sexual Boundary rules of the Board based on a sexual relationship with a patient. HIDALGO, Merlin Z., M.D.: License # 010556; 07/01/99. Licensee's Maine medical license was non-renewed. Licensee engaged in unprofessional conduct by knowingly giving false answers in his medical staff application to Cary Medical Center. Licensee also failed to disclose prior disciplinary actions to Cary Medical Center. Licensee shall receive a censure for unprofessional conduct due to committing fraud on his application to Cary Medical Center. HOLDREDGE, Timothy S., M.D.: License # 015004; 05/27/99. Licensee was issued a conditional permanent medical license subject to the same conditions and monitoring requirements as temporary license issued by Consent Agreement of 09/8/98. Substance abuse. JAIN, Arun K., M.D.: License # 013297; 09/14/99. In lieu of a hearing, the licensee agrees to accept a reprimand and a $1,500 fine from the Board due to unprofessional conduct based on fraud and deceit in application for renewal of his Maine medical license. KARTELL, James P., M.D.: License # 008005; 06/24/99. By Consent Agreement licensee agrees to voluntarily surrender his license to practice medicine in the state of Maine. The voluntary surrender shall remain in effect until final resolution of criminal matters pending against the licensee in the state of Massachusetts. LUCK, Christopher L., P.A.-C: License # PA-129; 12/20/99. In lieu of a hearing, the licensee is placed on probation for a period of one year from the date of the Consent Agreement. The licensee will prescribe only non-scheduled drugs for six months from the date of this Agreement. The licensee will pay a $1,500 fine. This action is based on inappropriate prescribing practices by the licensee. ONE YEAR PROBATION ENDED 12/20/00 LUCK, Gregory C., M.D.: License # 010063; 07/13/99. By Consent Agreement licensee agrees to withdraw his appeal of the Board's preliminary denial of his application to renew his medical license. Licensee's Maine medical license will be renewed in Inactive status. Licensee agrees not to practice medicine in Maine or in any other jurisdiction during the term of this agreement. Licensee will be subject to monitoring for substance abuse. MEYER, THOMAS L., M.D.: License # 012974; 10/12/99. In lieu of a hearing, the licensee agrees to withdraw his application for renewal of an Active status license and request renewal of his medical license in Inactive status. This action is based on disciplinary action taken by the New Hampshire Board of Medicine on 12/31/1997. RAUCH, Steven M., M.D.: License # 010729; 04/13/99. In lieu of a hearing, the licensee agrees that the status of his license will be changed from Active to Inactive effective the date Consent Agreement is signed. Licensee may not practice in Maine or any other state. If licensee applies for reinstatement of Active status he will provide documented evidence of at least one (1) year of recovery from substance abuse. Unprofessional conduct and habitual substance abuse. REED, Stephen D., M.D.: License # 007890; 12/14/99. In lieu of a hearing, the licensee agrees to accept a Warning and pay a $1,500 fine as discipline for unprofessional conduct. The licensee agrees to an automatic suspension of his license if the Maine Board of Licensure in Medicine receives verified information that the licensee has engaged in the unlawful practice of selling drug samples. The licensee further agrees that he will not supervise physician assistants as part of his medical practice. VIOLETTE, E. Jeffrey, M.D.: License #012065; 03/23/99. In lieu of a hearing, the licensee agrees to accept a reprimand, $1500 fine and random review of his records for a two-year period. Incompetence and unprofessional conduct based on prescribing issues and billing practices. WERNER, Donald R., M.D.: License #012869; 09/14/99. In lieu of a hearing, the licensee agrees to voluntarily surrender his medical license as of the date of the Consent Agreement. The licensee agrees not to seek to renew or reinstate his license to practice medicine in Maine. The signing of this agreement resolves all complaints pending before the Board, and the Board will dismiss the complaint pending in Administrative Court.
