ADVERSE LICENSING ACTIONS - 2001

PHYSICIAN/LICENSEE, EFFECTIVE DATE, DESCRIPTION


CLEMETSON, CHARLES D., M.D.: License # 013808; 6/12/01. At the conclusion of an Adjudicatory Hearing held on June 12, 2001 the Board voted to issue a censure for unprofessional conduct. In addition, Dr. Clemetson will be placed on probation for a minimum of one year, during which time he must hire an expert to implement changes in his office procedures.

HARRIS, VERNE D., M.D.: License # 014445; 10/09/01. At the conclusion of an Adjudicatory Hearing held on October 9, 2001, the Board voted to suspend the licensee's Maine medical license for a period of 360 days. This action is based on unprofessional conduct and incompetence.

HARVEY, JOHN E., M.D.: License # 014143; 01/09/01. In lieu of proceeding to an Adjudicatory Hearing the licensee agrees to accept a reprimand and participate in a sensitivity training program, to be approved by the Board, within one year from the effective date of this agreement. If the licensee fails to comply with this requirement, the Board will revoke his license. In addition, the licensee will send a letter of apology to the patient and her husband. This action is based on unprofessional conduct.

HOWELL, GEORGE B., M.D.: 014879; 04/10/01. At the conclusion of an Adjudicatory Hearing held on April 10, 2001 the Board voted to issue a reprimand for fraud and deceit in obtaining a license and to issue a reprimand for unprofessional conduct based on the licensee's failure to disclose discipline by other states.

JOZEFOWICZ, THADDEUS H., M.D.; 12/11/2001; Five year probation and conditions on license effective 12/11/96 ended on 12/11/01

LEACH, WILLIAM, M.D.: License #009644; 11/28/00. On September 12, 2000 the Board voted to preliminarily deny the licensee's application for renewal of his medical license. The licensee was notified by letter dated October 30, 2000, sent certified mail, return receipt requested. The licensee did not appeal the Board's action and the denial became final and effective November 28, 2000. This action is based on the licensee's failure to report actions taken by the Florida Medical Board. This action is listed in 2001 because it was not reported until that time.

OLSON, KAREN L., M.D.: License # 013116; 03/13/01. In lieu of proceeding to an Adjudicatory Hearing the licensee agrees to accept a warning from the Board for breach of patient confidentiality. This action is based on unprofessional conduct.

OLSTEIN, JOEL S., M.D.: License # 008973; 11/13/01. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees that the conduct which gave rise to the federal charges is also grounds for discipline for unprofessional conduct. The licensee agrees to accept a censure and pay a $10,000 fine.

SHEFFER, EMILY H., M.D.: License # EC-99-164; 09/10/01. In lieu of proceeding to an Adjudicatory Hearing, the licensee agrees to accept a reprimand for unprofessional conduct and incompetence for inappropriate prescribing practices. The licensee used patient names in order to obtain controlled substances for herself. The licensee will continue to participate in her recovery program with the Committee for Physicians Health (CPH) of the Medical Society for the State of New York. A violation of her agreement with the CPH will constitute a violation of this agreement and may result in further sanctions.

TORRES, RUDOLFO B., M.D.: License # 006462; 07/11/00. On July 11, 2000 the Board voted to preliminarily deny the licensee's application for renewal of his medical license. The licensee was notified by letter dated May 16, 2001, sent certified mail with return receipt requested. The licensee did not appeal the Board's action and the denial became final and effective on June 24, 2001. This action is based on the licensee's failure to report actions taken by the Florida medical board.

VIOLETTE, E. JEFFREY, M.D.: License # 012065; 07/10/01. At the conclusion of an Adjudicatory Hearing held on July 10, 2001 the Board voted to suspend the licensee's medical license for 360 days, effective 7/19/01. During the suspension period Dr. Violette shall have an evaluation to include neuropsychological testing, personality testing and a physical examination. The licensee shall engage in therapy/treatment with providers approved by the Board. Prior to the expiration of the 360 day suspension period, Dr. Violette must satisfy the Board that he is fit to practice medicine. If he receives his license to practice he shall not self-prescribe medications. This action is based on findings of unprofessional conduct and incompetence at the conclusion of the adjudicatory hearing.

WOODRUFF, ALAN F., M.D.: License # 006151; 07/25/01. In lieu of proceeding to trial in the District Court of Southern Kennebec County on a complaint alleging incompetence, unprofessional conduct and inappropriate prescribing practices, and seeking revocation of his license, the licensee agreed to voluntarily surrender his Maine medical license effective July 25, 2001.

WILSON, JAMES E. P.A.-C: License # PA-684.;10/19/01. In lieu of proceeding to an Adjudicatory Hearing the licensee agrees to accept a reprimand. In addition, the licensee shall only work in a supervised practice setting where his supervising physician is physically present. The licensee is subject to substance abuse monitoring and will continue with substance abuse treatment. This action is based on unprofessional conduct and inappropriate prescribing practices.
